As a Patient-Centered Medical home, we use a model of care that promotes care delivery centered around you as the patient. We’re recognized for our value-based approach to patient care, which is built on communication, care coordination, evidence-based quality measures, and resource stewardship. Integrating the PCMH model into our care has been known to promote the delivery of quality care, support chronic disease management, and strengthen the patient-provider relationship.
